Limited/Urgent Care Services Plan

When you purchase the Limited / Urgent Services Plan you will receive:

  • 3 Same Day or Urgent Care visits to your doctor.
  • 2 Diagnostic Services (x ray, pulmonary function test, EKG)
  • 3 Basic lab tests (Strep tests, Urinalysis)
  • Unlimited e-services (on-line communications with your doctor)
  • A 40% discount off all services provided by your doctor (except immunizations) 
  • $10 co-pay for office visits.

The cost for this plan is $25.50 per month.
