When you purchase the Chronic Conditions Plan you will receive:

• 3 Same Day or Urgent Care visits to your doctor  ($10 copay)
• 6 Diagnostic Services (x ray, pulmonary function test, EKG)
• Basic lab tests (1 lipid panel and one metabolic panel)
• Unlimited e-services (on-line communications with your doctor)
• A 40% discount off all services provided by your doctor (except immunizations) 

The cost for this plan is $65.92 per month.
