The Preventive Care Plan is intended for people who want to get an annual physical exam.  You expect to make an annual trip to your primary care doctor and want to be sure you are covered.

When you purchase the Preventive Care Plan you will receive:

• 1 Routine Office Visit ($10 copay)
• 1 Physical Exam with your doctor  ($10 copay)
• 1 Ultrasound or 1 CT  or 1 Stress Treadmill exam at our Diagnostic Center
• 2 Diagnostic Services (x ray, pulmonary function test, EKG)
• Basic lab tests (1 lipid panel and one metabolic panel)
• Unlimited e-services (on-line communications with your doctor)
• A 40% discount off all services provided by your doctor (except immunizations) 

The cost for this plan is $58.08 per month. This plan may be combined with other plans for more comprehensive care.
