Excluded Services and Clarification

The Cornerstone Medical Plan saves you money by carefully defining what is covered under each plan, to keep costs from growing past the point where you can afford it.  Please note the following exclusions and clarifications:

  • A Same Day or Urgent Care visit is limited to caring for just one ailment (i.e., a sore throat, inflamed foot, asthma attack).
  • A Routine Office visit is used to follow up on a chronic condition.
  • Diagnostic Services and Basic Lab Tests (x ray, pulmonary function test, EKG) are limited to those provided at your doctors office.
  • Cornerstone Medical Plan only covers services provided by the doctor you sign up with.  We are unable to provide coverage to specialists or other outside sources (such as emergency room, immunizations, referrals to outside sources, specialist visits, hospitalization, non-formulary drugs, lab testing or x-ray services which are not listed above). 
  • e-services include medication refills, referral requests and other communications which can be handled on-line.  Your doctor will not treat your medical condition on-line .
  • Services must be used within one year of purchase.
  • We can not apply the 40% discount to immunizations as we already set our fee to be right at the cost of the supplies and administration.
