Appointments
To schedule an appointment, call 313-359-0801. Well appointments (physicals, routine checkups) should be made a couple weeks in advance.For illnesses that require a same-day visit, please call first thing in the morning. If you become ill later in the day, please call as soon as you begin to think that an appointment may be needed.

School/Sports Forms
Often, forms must be completed before a child can be enrolled in school or day care, or participate in sports. Links to the current versions of these forms are shown below. Please be sure to fill in all patient/parent sections of the forms before bringing them to us to complete. We can usually finish the forms at your child's well visit. Patient Registration
Patient registration is required for new patients and every two years for established patients. To save time while visiting the doctor’s office, download and complete our forms at home and bring them with you to your appointment.
Hospital Care
If you need hospital or specialty care, we can ensure a seamless transition as our physicians are on staff members at Providence and Providence Park. In addition, our physician team includes individual “hospitalists” who are experts in the type and manner of care provided within hospitals. We, like most primary care physicians, depend on these “hospitalists” to look after you while you are in the hospital. While you may not know the hospitalist assigned to you when you are admitted to the hospital, rest assured your primary care physician will work with the hospitalist to assure that you receive the best care during your stay.
After-Hours and Emergency Care
Our goal is to see our patients in the office when they are ill, and we work hard to accommodate your needs for sick appointments. Even if the office is closed, please call the office and listen to the recording for our answering service number. When you call the service, you will be asked for your name and contact number, as well as a brief description of the issue. You will then either be referred elsewhere or one of our doctors will return your call.In the event of a true life-threatening emergency, call 911 or use the hospital emergency room closest to your location. Urgent Care and Emergency Services

Please remember that insurance rules and coverage varies, and some policies will not pay for visits to an urgent care center unless the problem turns out to be a true medical emergency. Other policies specify what urgent care center you must use. Be sure that you understand your family’s policy.

You are expected to pay any co-pays at the time of your visit. You will also receive monthly statements showing any amount outstanding. If you find yourself in a financial situation that makes it difficult for you to pay your bills on time, please let us know right away; we may be able to help you investigate sources of assistance, and can arrange a timed payment plan to help manage any existing balance.
Insurance coverage and rules vary widely. We will verify that your coverage is active at the time of your visit, but it is your responsibility to understand your policy. This includes what kinds of services are covered, what your co-pays and/or deductibles are, whether you need referrals to see specialists, and whether you are required to use certain facilities, etc.
