Services
Routine well child, sick child, immunizations, laceration repair, hearing and vision screening, tympanometry, pulse oximetry.
Our east office location includes an on-site blood-drawing station for obtaining bloodwork, and a diagnostic facility including general x-ray and ultrasound.
Also available to our patients is a diagnostic center at 21 Mile and Romeo Plank where a full array of diagnostic testing can be performed including lab testing, radiology procedures and the most up-to-date CT scans.
Appointments
All visits with the pediatricians are by appointment only. Appointments for well-child visits should be made 1 to 2 months in advance, but of course there will be many times when an illness or problem means you need an appointment on much shorter notice, and we will always do our best to accommodate those needs. When you know that your child is ill, please call first thing in the morning on the day you think the child needs to be seen. If your child becomes ill later in the day, please call as soon as you begin to think that an appointment may be needed. Even if you are not certain whether or not the child needs to be seen, call – our nurse triage team specializes in helping you make that decision.

Patient Registration
We require a completed Patient Registration Form at the first appointment for new patients and every two years for established patients. You can save time and hassle by completing this forms at home and bringing it with you to the office.

Authorization for Medical Treatment
An Authorization for Medical Treatment Form is needed if someone other than a parent or legal guardian is bringing the child to our office. Please fill out this form and send it with the non-parent. Unless you specify a certain date range, one form will allow that individual (for example, grandma or a nanny) to bring the child to our office until you revoke the authorization.

Vaccines for Children Program (VFC)
This state and federal tax-supported program makes vaccines available at no cost to children who qualify. (An administration fee of $16.75 per vaccine is due at the time of service, except under Medicaid plans.) Eligibility is determined by federal guidelines. Use these forms to determine whether your child is eligible for the VFC program.

VFC Parent's Eligibility Worksheet If your child is covered under any health insurance plan except Medicaid, print this form and use it to guide you as you call your insurer about vaccine benefits. Bring the completed form with you to your appointment.

Immunization Screening Questionnaire
This form is used when a child is coming to the office only for immunizations (not to see the doctor). If a non-parent will be bringing the child, please print and fill in the form, and send it with the child & adult. This form does need to be completed on the same day the immunizations are to be given.
Newborn and Hospital Care
All of our physicians are board-certified pediatricians and provide care in the hospital as well as the office setting. Care is provided for newborn infants and hospitalized patients at St. John Hospital & Medical Center. St John Hospital provides the full breadth of pediatric care including pediatric Emergency Room physicians, in-patient care, pediatric and neonatal intensive care units, and a large staff of pediatric sub-specialty physicians.One of our pediatricians sees patients at both locations on a daily basis.
After-Hours and Emergency Care
Our goal is to see our patients in the office when they are ill, and we work hard to accommodate your needs for sick-child appointments. But naturally, there will be times when a significant problem arises too late in the day for an office call to be arranged. We have made careful arrangements for someone to help you with such problems 24/7/365, even when the office is closed. First, call the office and listen to the recording for our answering service number. When you call the service, you will be asked for your name and contact number, as well as a brief description of the issue. You will then either be referred to the Eastside Pediatric Clinic (more information below) or one of our pediatricians will return your call.
In the event of a true life-threatening emergency, call 911 or use the hospital emergency room closest to your location. When the situation is less emergent, contact the doctor on call through the answering service, as described above, for advice.
The Eastside Pediatric Clinic (ESPC) is a children-only urgent care center located at the St. John center at 21000 East 12 Mile Road between Little Mack and Harper in St. Clair Shores. The ESPC has a very experienced triage nurse team to take your phone calls and help you determine whether your child can be cared for at home until our office opens, or needs to be seen promptly. If a visit is necessary, you may be seen by whichever St. John-affiliated local pediatrician is on duty that day. ESPC hours are usually from 5:30pm to 10pm Monday through Friday, Noon to 5pm on Saturday and holidays, and 9:30am to 5pm on Sundays. Their phone number is 586-498-3600. When our office is closed but the ESPC is open, this is the place to call for basic help and advice, provided by experienced pediatric nurses.
If the Eastside Pediatric Clinic is too far for you to go for after-hours care, St. John Health operates a variety of other Urgent Care centers. Remember - for pediatric-specific advice, please call the ESPC, even if you would prefer to have the child seen elsewhere.
Please keep in mind that insurance rules and coverage vary, and some policies will not pay for visits to an urgent care center unless the problem turns out to be a true medical emergency; other policies specify what urgent care center you must use. Be sure that you understand your family’s policy.
Billing and Insurance Information
Lakeview Pediatric participates in many insurance plans for your convenience. Please call the office to find out whether your plan is on the current list. In the vast majority of cases, if you provide us with your up-to-date insurance information, our central billing office can bill your policy directly for services. You are expected to pay any co-pays at the time of your visit. You will also receive monthly statements showing any amount outstanding, and we ask that you pay all outstanding amounts within 1-2 months of when they first appear on your statement. If you find yourself in a financial situation that makes it difficult for you to pay your bills on time, please let us know right away; we may be able to help you investigate sources of assistance, and can arrange a timed payment plan to help manage any existing balance.
Insurance rules and coverage vary widely. We will verify that your coverage is active at the time of your visit, but it is your responsibility to understand your policy: what kinds of services are covered, what your co-pays and/or deductibles are, whether you need referrals to see specialists, whether you are required to use certain facilities, etc.
The billing for Lakeview and other St. John-affiliated offices is handled by a central billing office. That office send out insurance claims, communicates with the insurance companies, and prepares and mails monthly statements. If you have questions about billing or statements, you should first call the billing office. However, if your questions are not answered to your satisfaction, don’t hesitate to call our Lakeview office supervisor.
