New Patient Information
Consultants in Cardiology PC
We accept new patients for cardiac and peripheral vascular consultation, diagnosis, and treatment. To assist our new patients with their first appointment, we offer the following helpful hints:
Prior to your appointment, please obtain and forward copies of your medical records.
Please arrive 15 minutes prior to your scheduled appointment.
While you will be pre-registered over the phone, it will still be necessary to complete paperwork at the time of your appointment, so please remember to bring your insurance card and picture I.D.
You will be required to sign a HIPAA Privacy Act Form at the time of your visit. If you have any questions regarding this form, please do not hesitate to ask.
It is recommended you write down any questions you may have for the physician in preparation for your visit.
Please arrive promptly at the time of your scheduled appointment and plan on being at the office for approximately one hour.
Please bring all of your medications, including over-the-counter remedies, with you for your appointment.
If you have a HMO insurance, you are responsible for obtaining the necessary referral. If your referral is not available at the time your visit, your appointment may need to be rescheduled.
If you have a co-payment, BCBS Master Medical, or no insurance, you will be responsible for payment at the completion of each visit. We accept Visa, Mastercard, check, or cash. If you have further billing questions, please refer to our Billing Site.
