Appointments/Office Hours 

Monday - Friday 8:30 a.m. to 5:00 p.m.

These are the normal office hours; however, your physician may see you before or after these hours depending on special circumstances.  Patients are seen by appointment only.  If you are unable to keep a scheduled appointment, please contact the office as soon as possible so that other patients waiting to see the physician can be placed in your time slot. After Hours Phone Calls

If an urgent situation should arise after our normal office hours, physicians can be contacted through our answering service at (248) 546-3194.  In an emergency situation, call 911 or go to your nearest emergency department.

Fees/Insurance

Our fees are the usual and customary charges for Physical Medicine in this area.  The majority of our physicians participate with Blue Cross, PPOM, and Medicare.  Please contact our office if you have a specific insurance question.  We will bill your insurance for you; however, you are responsible for any portion not covered by your insurance company.  Payment is expected for office services at the time the service is rendered.  Payment can be made by cash, personal check, Mastercard, Visa, and Discover.

Referrals

It is your responsibility to understand the requirements of your insurance company.  If you must obtain a referral from your primary care physician before seeing one of our specialists, please make sure that our office has a copy before your scheduled appointment.  The physician cannot see you without your required referral.

Medical Records

Your medical record will be kept confidential and will only be released if you authorize us to bill your insurance company or with your special authorization to another physician or other individual or group.  Under routine circumstances, we will not release your medical record without your consent.   In some rare situations, we must release your medical record upon receipt of a subpeona or cout order, if your treatment with us is related to some legal proceeding.  You have the right to know the names of anyone requesting or receiving this information.

Prescription Refills

Requests for prescription refills should be called in to the office at least two business days before you run out of your medication.  Normally, the refill can be called in within one business day; however, occassionally, the physician will want to speak with you or see you before the medication can be refilled.  Please call in your request as soon as possible to assure the continous efficacy of your medication.  Please safeguard medications from loss or theft.

While every care is taken to assure that your questions and concerns are addressed timely and appropriately, the Physicians and staff, of the Center for Physical Medicine and Rehabilitation, P.C., understand that occassionally you may have a special need or recommendation. Barbara Wingfield, Practice Manager, is employed by CPMR to address those needs.
