To schedule an appointment, call 313-343-7280. Appointments usually are scheduled about five weeks in advance.

Patient Registration

Patient registration is required for new patients and every two years for established patients. To save time while visiting the doctor’s office, download and complete our forms at home and bring them with you to your appointment.

After-Hours and Emergency Care

Our goal is to see our patients in the office when they are ill, and we work hard to accommodate your needs for sick appointments. Even if the office is closed, please call the office and listen to the recording for our answering service number. When you call the service, you will be asked for your name and contact number, as well as a brief description of the issue. You will then either be referred elsewhere or one of our doctors will return your call.

Urgent Care and Emergency Services

St. John Providence offers a variety of Urgent Care and Emergency centers. For an up-to-date list of locations, click here.

Please remember that insurance rules and coverage varies, and some policies will not pay for visits to an urgent care center unless the problem turns out to be a true medical emergency. Other policies specify what urgent care center you must use. Be sure that you understand your family’s policy.

Billing and Insurance Information

We participate in many insurance plans for your convenience. To verify your plan is accepted, please call us at 313-343-7280.

You are expected to pay any co-pays at the time of your visit. You will also receive monthly statements showing any amount outstanding. If you find yourself in a financial situation that makes it difficult for you to pay your bills on time, please let us know right away; we may be able to help you investigate sources of assistance, and can arrange a timed payment plan to help manage any existing balance.

Insurance coverage and rules vary widely. We will verify that your coverage is active at the time of your visit, but it is your responsibility to understand your policy. This includes what kinds of services are covered, what your co-pays and/or deductibles are, whether you need referrals to see specialists, and whether you are required to use certain facilities, etc.
